One of the commonest concerns we hear from our patients is about sagging jowls and a loss of definition in the jawline. A lot of patients pull back the excess skin to show the lift and definition they would like to regain in their lower face.
While dermal filler can work wonders in refining and restoring structure to the lower face, it's important to understand what it can and can’t do. Despite what some may claim, filler doesn’t ‘lift’ jowls in the way a surgical facelift would—but what it can do is blend them in beautifully, creating a smoother, more youthful contour.
How Filler Works to Improve the Lower Face
Jowls develop due to a combination of factors: volume loss, changes in skin elasticity, and the natural pull of gravity over time. You lose facial volume as the fat pads that support your skin deflate, while the bone underneath also shrinks. The skin that now has lost its supports shifts downwards & forwards, causing nasolabial (nose to mouth) lines with jowls and a sagging jaw line.
One key area that loses volume as we age is the lateral chin (the area either side of centre, below the mouth corners) This loss leads to a shadow effect, making the jowls look more prominent, and it also reduces support to the corners of the mouth, contributing to that downturned, sad-looking expression many patients want to correct.
By strategically replacing lost volume in the lateral chin, as well as along the jawline itself, we can restore support to the lower face and create a more harmonious transition from the jawline to the chin. This helps soften the appearance of jowls without the need for excessive product or an unnatural ‘overfilled’ look. When done correctly, this treatment enhances the flow of the lower face, making the overall structure appear more balanced and youthful.
The Importance of Facial Balance
While treating the jawline directly can be effective, it’s crucial to consider the face as a whole. If a patient has experienced significant volume loss in the midface (cheeks), addressing this first is often the best approach. The cheeks provide foundational support for the lower face, and restoring volume here can indirectly improve the jawline’s appearance—sometimes even reducing the need for additional treatment in the jowl area.
Similarly, volume loss in the preauricular (in front of the ear) and temple areas can contribute to a hollowed, aged appearance. Carefully placed filler in these areas can restore facial structure, providing subtle yet powerful support to the lower face.
